New Patient Forms
Your First Visit
Prior to your first visit, you will want to download and fill out our new patient forms, which can be found below. This saves you time at your initial visit so we can get right to providing you with excellent quality care! After your forms have been collected and sorted out, we will go over the details together to determine your care needs and goals.
The first visit to our offices typically includes an X-ray that allows us to view the structure of your jaw, the position of any teeth that have not yet erupted, malformed roots and tooth decay. This allows us to all be on the same page about your existing oral health before we move forward with any kind of treatment.
To that same end, this initial visit will also involve a review of your medical history. When you share your medical history, be sure to provide complete, comprehensive and up-to-date information about your health. Some medical history factors of particular note include recent hospitalizations or surgeries, as well as any recent illnesses. Additionally, you will want to provide your dentist with the names, doses and frequency of any medications you’re taking, both over-the-counter and prescription, and the name of your physician.
If any changes in your health or medications occur during your treatment process, let us know! This information will help us to select a treatment method that is the most effective, comfortable and safe for you.
Regular Checkups
We encourage regular dental checkups for individuals ages three and up. These checkups include a thorough cleaning, polishing and oral exam by one of our expert dentists. During this exam, your dentist will check on your existing fillings and restorations, as constant pressure from chewing, grinding or clenching may have caused them to wear, chip or crack over time.

Generally, these regular checkup appointments take around an hour to complete. Patients should schedule these appointments in advance, as patients are seen by appointment only. We recommend scheduling your next appointment each time you come in for these regular checkups to make sure you don’t forget!
At each appointment, we make an effort to be on time for our patients and appreciate when patients extend that same courtesy to our team. If something does come up and you are unable to make an appointment, we understand! Simply make sure to notify us immediately so we can adjust or reschedule your appointment.
